It doesn’t seem to end…

So, I’d filled out a form, enclosed a death certificate, and mailed it out to the life insurance company on the policy I didn’t know he had.

I get back yet another form requesting:

1.  Another copy of his death certificate

2. Copy of police report/accident report/EMT report.

3. Copy of medical examiner’s complete investigative report

4. Copy of the autopsy and toxicology report (if available)

5. Copy of hospital records (if applicable)

6. Signed health History and HIPPA Authorization forms

7. if accident was due to a fall….complete the fall questionnaire (2 pages) and return with health history and HIPPA forms.

8. Copy of his account statement for the month of his death.

*note: Upon claim review by carrier, additional documentation may be requested.
